Patient Registration


The information contained in steps 1 thru 5 will help prepare you for your upcoming visit to Weston Endodontic Care. The final tab allows you to complete your registration online at your convenience. The registration process is web-encrypted and secure allowing you to fill out the registration in the comfort of your home. The entire process should take less than 10 minutes. You will need a password to complete your registration and we will provide it to you during your first contact with our office. 1) What to Expect

We will treat you like family. The patient-doctor relationship is built upon mutual trust and communication. Dr Limosani believes in a patient centered approach to dental care. Our goal is for you to feel confident and comfortable with our approach. In some instances no endodontic treatment is recommended.

Dr Limosani will assess your dental condition and provide you with guidance in order for you to satisfy your oral health needs.

Our staff will explain all fees and work to maximize any insurance benefits.

We will work as a team with your general dentist’s office to provide you with the best care possible.

2) What to Bring

Please be sure to bring the following items with you to your appointment:

The referral slip and any x-rays from your dentist’s office.

A list of medications you are presently taking.

Any necessary dental insurance information if you have insurance.

Drivers License or Photo ID.

3) Your First Visit

When you call to schedule your appointment, we will email you a welcome letter that will provide you with a password to complete your online registration. Upon arriving in our office you will digitally sign the forms you reviewed online.

Dr Limosani and our team will take any necessary radiographs and photographs in order to thoroughly evaluate your condition and make a proper diagnosis.

Dr Limosani will discuss all findings, show you radiographs and photographs, and make recommendations. Patient education is at the forefront of our objectives.

Sometimes treatment may be rendered the same day. Please note that many endodontic treatments take more than one visit depending on the complexity of your condition. Our goal is quality care.

After treatment, your case and prognosis will be reviewed and post-operative instructions will be given.

We will communicate with your dentist regarding any findings, recommendations as well as outline any treatment rendered.

4) Payment and Insurance

Endodontic services vary greatly due to the nature and complexity of the procedure. The fee for your endodontic treatment will be based on the extent of treatment. During your first visit we will discuss the probable number of visits, their length, and the fees involved.

We understand that a significant number of our patients rely on dental insurance to cover most of their dental needs. Our staff is committed to working with you and your insurance company to maximize your insurance benefits. Our policy is to have our patients with insurance pay one half of treatment cost at the time of treatment. If your insurance company reimburses at a higher rate, we will provide you with a refund. Occasionally insurance companies will not cover the entire remaining balance. When this occurs you will be responsible for the balance.

Our practice also participates with a financial assistance company, Care Credit. For your convenience, Care Credit applications can be accessed and completed either online or in our office. Please contact our office for details.

6) Electronic Registration
